The Family Place MKE is in process of becoming an in-network provider for the following insurance companies: Optum (UHC/UMR), Anthem Blue Cross Blue Shield, Network Health and Aurora Advocate.

 

There can be many reasons you may decide not to use your health insurance benefits such as seeing an Out of Network Provider, privacy or cost. For those who make that decision our out-of-pocket (private pay) rates are as follows.

• Adult or Teen Individual Therapy (60 minutes) $150 per session

• Play Therapy (45 minute) $125 per session

• Family Therapy (60 minute) $200 per session

 

A super bill can be provided on request if you wish to use Out of Network Benefits or for FSA/HSA/HRA documentation.

No Surprises Act and Good Faith Estimates

Under the law, health care providers need to give patients who don’t have insurance or who are not using insurance an estimate of the bill for non-emergency medical items and services. If you would like an estimate for costs of services, please contact us. If you are using your health insurance plan, an estimate can also be provided but we strongly recommend contacting your health plan directly to verify benefits if you are unclear on what your patient financial obligation for services may be.
